Portsmouth - OT - Community Rehab Service

We are currently seeking an experienced Occupational Therapist to join the Community Rehab Service in Portsmouth on a temporary basis. This integrated service, run in partnership with Portsmouth City Council and Hampshire and Isle of Wight Healthcare, supports individuals post-hospital discharge, or following a fall, health crisis, or change in condition-helping them regain independence and avoid further health decline.

Location: Portsmouth (Based at St Mary's Campus with travel across the city)
Rate: Negotiable, dependent on experience
Contract Type: Temporary / Locum

Our Mission:
To assist people of all ages with physical, mental, and social impairments to optimise their functional independence within their homes and communities.

Key Responsibilities:

Complete assessments, set patient-centred goals, and develop tailored reablement plans

Work with complex cases involving individuals with significant health and social care needs

Deliver interventions in patients homes, promoting independence and quality of life

Use assistive technologies and recommend/adapt methods to support daily functioning

Prescribe, demonstrate, and guide use of specialised equipment

Provide practical and emotional support to individuals and their families/carers

Manage your own caseload independently, while contributing to a highly supportive team environment

Ensure case recording is maintained in line with professional and local standards

Maintain up-to-date knowledge and application of relevant legislation and safeguarding procedures

Essential Requirements:

HCPC-registered Occupational Therapist

Substantial post-qualification experience, ideally within community or reablement services

Strong understanding of statutory frameworks including the Care Act and safeguarding policies

Ability to work flexibly, independently, and collaboratively across disciplines

Excellent communication, documentation, and interpersonal skills
