We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work Community Team in Bristol. This role is pivotal in delivering high-quality support and interventions to adults in need, ensuring they have access to the services and resources that empower them to lead fulfilling lives. As a member of our dynamic team, you will work collaboratively with individuals, families, and other professionals to promote well-being and safeguard vulnerable adults within the community.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals' needs and risks.
  • Developing and implementing care and support plans in collaboration with service users and their families.
  • Providing ongoing support and monitoring the effectiveness of interventions.
  • Working closely with healthcare providers, community organizations, and other stakeholders to ensure holistic care.
  • Maintaining accurate and up-to-date records in accordance with local policies and legislation.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Current registration with Social Work England.
  • Strong knowledge of relevant legislation and best practices in adult social care.
  • Excellent communication and interpersonal skills, with the ability to build rapport and trust with service users.
  • Proven ability to work effectively within a multidisciplinary team.
