Social worker-Young Adults Transitions - Bristol - Hybrid
A Social Worker is required for work within the Adults Transition Team in the vibrant city of Bristol. Excellent rates of pay up to £32ph. 3 month minimum contract but likely extended. 37 hours per week. Exciting opportunity in a great city with good transport links to neighbouring authorities.. Working pattern: Full-time. 2x days per week are mandatory in-office at City Hall (Wednesday plus one other day) working from home is available on other days.
Role is a specialist social work team supporting young adults age 16-25 who are transitioning from childrens social care to adult social care. Practitioner is expected to have good working knowledge of the Care Act, Mental Capacity Act, best interest processes and safeguarding processes (including managing s42 enquiries). The practitioner would only be a case holder with adults, but would be expected to work alongside childrens social care professionals.
The social worker will be responsible for providing professional social work, supporting adults with care and support needs and their carers undertaking a range of statutory duties in accordance with relevant legislation.
They will assess the needs of people and their carers, will plan, implement, review, and evaluate outcomes for and with individuals. The social worker will ensure the principals of choice and control are used in supporting people to make their own decisions about the way they organise their support and how to manage the risks involved.
They will demonstrate commitment to strengths-based practice and skills in supporting people to identify and build support networks to achieve their identified outcomes.
You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
