Job description

Locum Social Worker (Mental Health) - Islington Assertive Outreach Team

Work in a multi-disciplinary mental health team alongside colleagues employed by the NHS and other agencies.

Managing a caseload of service users suffering from mental illness and some with co-existing substance misuse.

Provide a range of interventions either directly or through others to address service users mental health needs.

To improve the service user's engagement with services and reduce hospital admissions.

To increase stability in the lives of service users and their carers/families.

To improve social functioning.

To promote the service users participation in their own recovery and through educational support to develop coping strategies over symptoms of mental illness.

To take responsibility for promoting and safeguarding the welfare of young person's /vulnerable adults in your care and those that you come into contact with.

Undertaking assessments of service users mental health needs as part of a multi-disciplinary team.

Undertake assessments under the Care Act.

Acting as care coordinator for the purposes of care programming and statutory aftercare.

Participating in duty rota as required.

Supporting service users with mental health problems to access support from other agencies and informal networks.

Making referrals as necessary to children's social care services when children in need or child protection issues are identified.

Attendance at meetings, case reviews, conferences, working parties and training when required.

Giving evidence at Mental Health Review Tribunals and Managers' hearings as required.

Carrying out an adequate level of workload recording, using electronic recording systems as necessary.

Preparation to written reports when required.

Person Specification

Professional Social Work qualification and registration.

Considerable experience of mental health work in a community setting.

Ability to engage with mental health service users and carers/families.

Knowledge of relevant social care and mental health legislation, national standards, policy and practice and current issues in safeguarding and the provision of services for people with psychosis.

Proven ability to liaise effectively with partner agencies, independent sector and voluntary providers in Islington in providing social work assessments and care management services to people with psychosis.

Proven ability to make assessments of people with psychosis incorporating multi-disciplinary input into assessments and formulating multi-agency plans.

Demonstrable ability to undertake safeguarding investigations and co-ordinate agreed plans, with support and supervision.

Demonstrable ability to undertake effective care planning for people with psychosis within a recovery model and using a strengths approach.

Demonstrable ability to provide clear and accurate case recording using electronic recording systems. Devise reports and present reports for Mental Health Act Tribunals and reports to panels and other formal settings.

Proven ability to communicate effectively and accurately both verbally and in writing e.g. Tribunal and Court reports, planning meetings, reviews and personal budget documentation.

Able to demonstrate effective interpersonal skills through working well with others, being confident with any advice given and by engendering trust and confidence in others.

Able to form and maintain appropriate professional relationships and boundaries with mental health service users and their carers.

Proven ability to plan and prioritise workloads and meet deadlines.

Hours: Monday to Friday, 9am to 5pm

Ideally to start beginning March.
