Eden Brown Synergy is currently recruiting on behalf of a Local Authority in Tower Hamlets for an experienced SEN Preparing for Adulthood Officer to join their Children and Culture Directorate, specifically within the Education and Partnerships service.

This opportunity is ideal for professionals with a strong background in SEN and transition work, and who have extensive experience within Preparing for Adulthood (PfA) teams, supporting young people aged 14-25 with complex needs.

Key Responsibilities:
* Act as the key point of contact for young people aged 14-25 with complex learning difficulties and/or disabilities, supporting their transition into further education, training, or employment.
* Carry out person-centred Education, Health & Care (EHC) assessments and ensure recommendations reflect educational, health, and care support needs.
* Lead and coordinate transition planning (starting from Year 9), ensuring long-term, outcome-focused placement strategies.
* Provide information, advice, and guidance (IAG) to young people and their families regarding transition pathways.
* Monitor placements and challenge provisions if needs are not being met appropriately.
* Maintain accurate placement/progress records, contribute to placement panels, and manage personal budgets where applicable.
* Collaborate across borough services (health, education, social care) and external partners to deliver integrated support packages.

Requirements:
* Significant experience in Preparing for Adulthood (PfA) roles or transition teams.
* Strong understanding of EHCP processes, person-centred planning, and post-16 SEN provision.
* Sound knowledge of legislation relevant to SEN, including the Children and Families Act, Care Act, and SEND reforms.
* Experience working with young people with complex needs and coordinating multi-agency support.
* Excellent organisational, communication, and IT skills.
* Enhanced DBS required (Adults & Children).

Desirable Skills:
* Knowledge of post-16 funding structures and personal budget implementation.
* Familiarity with the Mental Capacity Act in relation to post-16 young people.
* Ability to run workshops and provide training for professionals and parents.
