Location: Bromley Council
Rate: £37.52 per hour
Contract: 3 months

Bromley Council is seeking an experienced Locum Social Worker to join the Preparing for Adulthood Team on a 3-month contract.

The role involves working with young people transitioning from Children's to Adult Services, ensuring that their care and support needs are met under the Care Act 2014. The successful candidate will need to demonstrate experience in adult social care, transition planning, and multi-agency working.

You will be responsible for:

Carrying out Care Act assessments and reviews

Developing outcome-focused support plans

Working closely with education, health, and other partner agencies to ensure a smooth transition into adulthood

Advocating for service users while promoting independence and choice
