Location: Southwark, London
Pay Rate: £33.76 per hour
Contract: Locum, Full-Time
Working Pattern: Hybrid

Southwark Council's Intermediate Care Service (ICS) is looking for a proactive and experienced Qualified Social Worker to support adults awaiting Care Act assessments or reviews. You will work as part of a multi-disciplinary reablement service focused on promoting independence and recovery.

Key Responsibilities:

Complete Care Act assessments, reviews, and person-centred support plans

Undertake Mental Capacity Assessments and chair Best Interests meetings

Provide crisis intervention and timely signposting to community services

Support individuals with complex social and health needs

Work closely with health professionals and informal carers

Promote strengths-based practice and positive risk-taking

Requirements:

Registered with Social Work England

Experience in reablement or short-term intervention services

Strong understanding of the Care Act 2014 and MCA

Skilled in assessment, planning, and partnership working

The role provides an exciting opportunity to work in a multi-disciplinary setting, with a focus on strengths-based practice and promoting independence and recovery.

The main duties of the role include goal setting, completing Care Act assessments and devising care and support plans, completing reviews, completing Mental Capacity Assessments, chairing Best Interests meetings, crisis intervention, signposting, providing advice and information and working with people with complex social needs.

There is also a Safeguarding function within the service, which includes completing initial fact finding and assessing and minimising risk.

Our Intermediate Care Service (ICS) is seeking a Social Worker to work within the Reablement Service. This role offers a unique opportunity to thrive in a multi-disciplinary environment, collaborating closely with health professionals. We place a strong emphasis on strengths-based practice, crisis management, independence, and recovery, ensuring that each individual receives a comprehensive support tailored to their needs.

The role encompasses a diverse range of tasks, including:

Providing advice and information to service users and their families

Goal-setting for reablement and rehabilitation

Conducting Care Act assessments and person-centred care and support plans

Facilitating reviews and Mental Capacity Assessments + Best Interests meetings

Delivering timely signposting to necessary community resources

Working closely with individuals with varied health and social care needs, including informal carers

Conducting initial safeguarding fact-finding, risk assessment, and mitigation
