Role Summary:
An exciting opportunity has arisen for a Band 6 Occupational Therapist to join the D2A (Discharge to Assess) team at Musgrove Park Hospital. This is a community-facing role focused on supporting adults recently discharged from hospital into their own homes, with the aim of promoting independence and avoiding unnecessary readmission.

Key Responsibilities:
Undertake community-based Occupational Therapy assessments and interventions for adults post-hospital discharge

Work collaboratively with D2A provider agencies to deliver short-term reablement packages

Set patient-led SMART goals to support progression toward independence

Identify and recommend longer-term care and support needs based on clinical assessment

Engage in multidisciplinary discussions to ensure coordinated and holistic patient care

Maintain accurate and timely clinical records in line with Trust policies

Essential Requirements:
HCPC-registered Occupational Therapist

Proven experience in community rehabilitation and/or adult reablement

Strong understanding of discharge pathways and risk management in the community

Full UK driving licence and access to a vehicle (essential due to travel requirements)

Willingness to work across geographical clusters and travel cross-county as needed

Additional Notes:
The service operates across multiple localities flexibility to meet patient needs across the region is essential.

An exciting opportunity has become available for an Occupational Therapist to join a developing neurobehavioural rehabilitation service in Cheadle, Staffordshire. The service supports adults with acquired brain injuries and complex needs, delivering highly specialist care in a therapeutic, outcome-focused environment.

You will work as part of a transdisciplinary team, delivering evidence-based interventions, supporting goal setting with service users and carers, and helping to maximise independence and quality of life.

Key Responsibilities:

Conduct neurological assessments and rehabilitation planning

Deliver interventions to support function, ADLs, and cognitive rehab

Assess environmental and equipment needs, including seating

Use clinical outcome measures (FIM/FAM, WHIM, CRS-R, TOMS)

Support complex discharge planning and liaise with community services

Supervise OT assistants and students

Contribute to the delivery and development of evidence-based practice

Essential Criteria:

HCPC-registered Occupational Therapist

Strong clinical knowledge in neurorehabilitation or complex needs

Experience working within MDTs and using outcome measures

Skilled in planning and evaluating treatment interventions

Ability to manage a designated caseload independently
