Lincolnshire County Council is seeking an experienced and motivated Qualified Social Worker to join our Community Adults Team, working specifically with individuals with physical disabilities and older adults.

We are looking for professionals who have a strong background in community-based adult services. This role is not suitable for those whose primary experience lies in hospital discharge, mental health, learning disabilities, or safeguarding teams.

Key Responsibilities:
Undertake strengths-based community assessments for adults with physical disabilities and older people.

Develop and manage person-centred support plans that promote independence and improve outcomes.

Engage with service users, carers, families, and multi-agency partners to ensure holistic care delivery.

Make informed decisions around eligibility and support provision in line with the Care Act.

Monitor and review care plans to ensure needs are met effectively and efficiently.

Maintain accurate, up-to-date records and documentation using Mosaic (experience with this system is desirable).

Requirements:
Essential:

Social Work qualification (CQSW, CSS, DipSW or equivalent)

Registered with Social Work England

Proven experience working in community adult services - specifically with physical disabilities and older adults

Strong understanding of the Care Act, personalisation, and community-based support

Excellent assessment, communication, and case management skills

Ability to work independently and manage a complex caseload

Desirable:

Experience using Mosaic case management system

Knowledge of Lincolnshire's local resources and adult care services
