Are you a dedicated and compassionate social worker looking to make a meaningful impact in the lives of adults within the North and West Community Team 2? We are seeking a qualified Social Worker to join our dynamic team, where you will play a vital role in supporting individuals and their families through various challenges. This position offers an opportunity to work collaboratively with a range of professionals to deliver high-quality social care services.

As a Social Worker in our team, your key responsibilities will include:

Conducting comprehensive assessments to identify the needs of clients and developing tailored care plans. Providing ongoing support and advocacy for individuals, ensuring their voices are heard and respected. Collaborating with healthcare providers, community organizations, and other stakeholders to facilitate access to resources and services. Maintaining accurate and up-to-date records in compliance with legal and organizational standards. Participating in team meetings and contributing to the continuous improvement of service delivery.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with the relevant professional body (e.g., Social Work England). Demonstrated experience in adult social care, including knowledge of relevant legislation and best practices. Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues. Excellent organizational skills and the ability to manage a diverse caseload effectively.

We are seeking a dedicated and compassionate Adult's Social Worker to join our dynamic Community Team in Bristol. As a social worker in Adult Social Care, youll play a crucial role in assessing and addressing the diverse needs of our community, navigating the varied challenges our vibrant city provides. Your responsibilities will also include:
