We are currently seeking a dedicated and experienced Adult's Social Worker to join our Community Team in Cardiff. As a vital member of our team, you will be responsible for providing high-quality social work services to vulnerable adults in the community.

Key responsibilities and duties include:

Conducting assessments and developing care plans for adults in need of support Working collaboratively with multi-disciplinary teams to ensure holistic care and support Providing advocacy and support to empower individuals to make informed decisions Monitoring and reviewing care plans to ensure they meet the changing needs of service users Participating in safeguarding processes and taking appropriate actions to protect vulnerable adults

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with current registration with Social Care Wales Demonstrated experience in working with vulnerable adults in a social work setting Strong assessment and care planning skills Excellent communication and interpersonal skills Ability to work effectively under pressure and manage competing priorities
