A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Strong knowledge of adult safeguarding and relevant legislation. Excellent communication and interpersonal skills. Ability to work effectively in a fast-paced environment and manage competing priorities.

Job Title: Qualified Social Worker - Hospital Discharge
Location: Portsmouth
Pay Rate: £36.01 per hour

We are currently recruiting for a Qualified Social Worker (QSW) to join the Hospital Discharge team in Portsmouth.

You will be mainly based on _The Limes_ ward, which is located within St James Hospital - Milton. In this role, you will be responsible for managing a caseload of patients and their discharge. This could involve community options or sourcing placements.

Requirements:

A driving license and own vehicle are required for this role.

You will be supporting placement visits in the community, which are not always within the Portsmouth area.
