We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Community Team in Slough. Your key responsibilities will include:

Conducting comprehensive assessments of individuals’ needs and risks. Developing, implementing, and reviewing care plans in collaboration with clients and their families. Providing ongoing support and guidance to clients, ensuring they have access to necessary resources and services. Working alongside multidisciplinary teams to coordinate care and enhance service delivery. Maintaining accurate and up-to-date case records in compliance with relevant policies and procedures. Advocating for clients’ rights and ensuring their voices are heard in decision-making processes.

The ideal candidate will possess the following qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Current Social Work England registration. Proven experience in adult social work, particularly in a community setting. Strong assessment and intervention skills, with a person-centered approach. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Sound knowledge of relevant legislation and best practices in social work.
