We are seeking a dedicated and compassionate Adult's Social Worker to join our Learning Disabilities (LD) Team in Walsall. This is an exciting opportunity for a qualified professional to make a real difference in the lives of adults with learning disabilities. You will be responsible for assessing needs, developing care plans, and providing ongoing support to individuals and their families, ensuring that they receive the best possible care and resources available.
Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and strengths. Developing and implementing individualized support plans in collaboration with clients and their families. Providing direct support and guidance to clients in accessing services, benefits, and community resources. Maintaining accurate and up-to-date records in compliance with local and national policies. Collaborating with multidisciplinary teams to ensure holistic care and support. Advocating for clients' rights and promoting their independence and well-being.To be considered for this position, you must possess:
A relevant social work qualification (e.g., Degree in Social Work, CQSW, DipSW). Current Social Work England registration. Experience working with adults with learning disabilities in a social work setting. Strong interpersonal and communication skills, with the ability to build rapport with clients and their families. Role Overview:
We are seeking a Social Worker to join the Learning Disabilities Team in Adults Social Care. Please ensure you complete the Adults Cover form to assist with MOC.
Main Purpose of the Role:
To improve outcomes for adults with care and support needs and carers.
To undertake effective strengths-based assessments, planning, and direct work with adults with care and support needs and carers that identifies needs in accordance with the Care Act 2014, statutory guidance, policy, and procedures.
To undertake safeguarding duties as required to protect adults with care and support needs from abuse or neglect.
To practice social work in a transparent, accountable, and safe way in accordance with professional standards, ethics, and values.
