As a Qualified Social Worker in the Leaving Care Team, your key responsibilities will include:
Providing high-quality assessments and support plans for young people aged 16-25 who are leaving care. Building trusting relationships with young people to empower them in their journey towards independence. Collaborating with external agencies and stakeholders to facilitate access to education, employment, and housing opportunities. Monitoring and reviewing care plans regularly to ensure the needs of each individual are met. Advocating for the rights and interests of young people, ensuring their voices are heard in decision-making processes.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (Degree in Social Work or equivalent). Current registration with Social Work England. Experience working with children and young people, particularly those in care or leaving care. Strong communication and interpersonal skills, with the ability to engage effectively with young people and their families.
