We are seeking an experienced and dedicated Qualified Social Worker (QSW) to join our Adult Learning Disabilities Social Work Team, based at Wessex House in Stockton-on-Tees. You will be working with adults with learning disabilities and their carers, providing person-centred, strengths-based social work support in line with statutory requirements and best practice.
Key Responsibilities
Deliver a high-quality social work service to adults with learning disabilities and their carers.
Undertake comprehensive Care Act assessments, including risk assessments and safeguarding referrals where appropriate.
Promote personalisation and independence through strengths-based support planning.
Commission and coordinate care packages, ensuring they meet assessed needs within budgetary frameworks.
Regularly review care and support plans, reassessing needs and making adjustments where necessary.
Complete Carers Assessments and support planning.
Maintain accurate case records and statutory documentation in line with local and national standards.
Participate in statutory duties such as court work, Mental Health Review Tribunals, and report writing.
Work collaboratively with colleagues, managers, and multi-agency partners.
Support service development by feeding back observations and improvement suggestions to management.
Act as a Practice Educator for social work students (minimum once every two years).
Take part in the duty rota, and if qualified, the AMHP or BIA rota.
Requirements
Qualified Social Worker with Social Work England registration.
Experience of working with adults with learning disabilities.
Strong knowledge of the Care Act 2014, Mental Capacity Act, Safeguarding, and DoLS.
Skilled in assessment, care planning, risk management, and inter-agency working.
Experience or willingness to act as AMHP or BIA (desirable).
Commitment to continuous professional development and supervision.
