Eden Brown synergy are currently recruiting for Herefordshire Council who are looking for Senior Practitioner Social Worker for Continuing Health Care


Herefordshire Adult Social Care is looking for an experienced CHC Senior Practitioner to undertake Joint Assessments / Reviews with our ICB to assess eligibility for CHC and Joint Funding. Must have a strong working knowledge of eligibility and process under the 2022 National Framework for NHS Continuing Healthcare and the Care Act 2014. Experience of undertaking CHC Checklists, Decision Support Tools and undertaking challenges and disputes regarding CHC Eligibility and Joint Funding with the ICB.



You will be familiar DST domains and experienced in obtaining evidence to support presenting argument based on Nature, Intensity, Complexity and Unpredictability of health need. You will also have excellent communication, presentation and negotiation skills.



Herefordshire Adult Social Care uses the Mosaic client database so experience with this system would be an advantage, but training will be provided as required. You will support the CHC Lead with the overall management of the CHC pathway from referral to outcome and monitoring the financial implications of those outcomes.



We are currently working a hybrid system of part office/part home-based working. There is a requirement to be able to work face to face and be based in the office in Hereford for at least 2 days each week. You will be given autonomy in the booking and confirming of visits for the days in office and then writing-up on the work from home days.

The Role:
We're currently seeking an experienced Adult Safeguarding Social Worker to join the Safeguarding Triage Team in Herefordshire Council. This hybrid role involves screening safeguarding concerns raised via the Advice and Referral Team (ART), gathering proportionate information, and making timely decisions based on Section 42 criteria.

You will be joining a highly supportive and knowledgeable team, based at Plough Lane, with access to experienced professionals and strong multi-agency partnerships.

Key Responsibilities:
Screen and assess safeguarding concerns to determine S42 thresholds

Gather relevant and proportionate information to make timely decisions

Engage directly with service users, ensuring Making Safeguarding Personal principles are followed

Work collaboratively with partner agencies and professionals across disciplines

Maintain accurate and high-quality case recordings and reports

Contribute to a person-centred approach that promotes safety, dignity, and independence

About You:
Qualified Social Worker with Social Work England registration

Extensive experience in adult safeguarding

Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, and related statutory guidance

Comfortable working in a hybrid setup with the ability to travel around Herefordshire

Strong interpersonal, assessment, and decision-making skills

Ability to work flexibly and autonomously in a dynamic environment
