We are seeking a dedicated and compassionate Adult Social Worker to join our Mental Health Team (MHT) in Oldham. This role involves supporting adults with mental health needs to live well and safely in the community.
Key Responsibilities:
Undertake Care Act assessments to identify the needs of individuals.
Arrange and monitor care packages, ensuring their effectiveness and appropriateness.
Monitor mental health and support people to remain well and maintain independence.
Work collaboratively as part of a Multi-Disciplinary Team (MDT) alongside health colleagues to manage risk.
Maintain accurate and timely records on the Paris system.
Use Mosaic system for safeguarding and purchasing care packages.
What We're Looking For:
Qualified and registered social worker (HCPC registered).
Experience or interest in mental health social work.
Knowledge of the Care Act and safeguarding procedures.
Ability to work effectively within a multi-agency team.
Excellent communication and organisational skills.
Commitment to person-centered, recovery-focused practice.

Oldham Council is seeking an Experienced Social Worker to join our Adult Social Care service on a peripatetic basis. This dynamic role is ideal for a flexible and skilled practitioner looking to make a meaningful impact across multiple teams, with a primary focus on safeguarding (SG) within the MASH team.
Key Responsibilities:
Undertake safeguarding enquiries and assessments within the Multi-Agency Safeguarding Hub (MASH).

Work across other ASC teams as required, providing support wherever operational demand is highest.

Requirements:
