Stockport Council is seeking an experienced Qualified Social Worker to join the South Neighbourhood Team, supporting adults aged 18+ under the Care Act 2014. This is an excellent opportunity to join a supportive team with flexible working and a focus on delivering high-quality, person-centred care.

You will be working with a varied caseload, carrying out assessments, reviews, and safeguarding work under Care Act duties. The role requires confident use of Liquid Logic and experience in Adult Social Care (ASC).

Key Responsibilities:
Undertake Care Act assessments, reviews, and support planning

Manage a caseload of adults aged 18+ with varying needs

Work collaboratively with health, housing, and voluntary sector partners

Carry out safeguarding enquiries in line with local and national guidance

Use Liquid Logic to maintain accurate, up-to-date case records

Promote independence through strengths-based approaches

Provide timely and appropriate interventions that meet statutory responsibilities

Requirements:
Qualified Social Worker with Social Work England registration

Minimum of 2 years' experience in Adult Social Care

Strong working knowledge of the Care Act 2014

Proficient in Liquid Logic case management system

Able to work independently and within a multidisciplinary team

Enhanced DBS (on the Update Service or willing to complete one)

Benefits:
£30 per hour (weekly pay via umbrella)

Hybrid working - only 2 days per week in the office

We are currently seeking an experienced and motivated Mental Health Social Worker to join the Hospital Discharge Team in Stockport on a hybrid working basis. This is a fantastic opportunity to work in a supportive and forward-thinking adult mental health service.

As a Hospital Discharge Social Worker, you will play a key role in supporting timely and safe discharges from mental health inpatient settings. You'll be working collaboratively with health and community teams to ensure individuals receive person-centred care and are supported through recovery and reintegration into the community.

Key Responsibilities:
Manage a caseload of adults with mental health needs, focusing on discharge planning

Carry out strength-based assessments and risk assessments

Develop and review support plans in line with Care Act and Mental Health Act frameworks

Liaise with multi-disciplinary professionals to coordinate services

Advocate for service users and uphold safeguarding principles

Ensure accurate and timely recording of case notes and reports

Requirements:
Qualified Social Worker with valid Social Work England registration

Previous experience in mental health hospital discharge is essential

Strong working knowledge of the Care Act, Mental Health Act, and safeguarding procedures

Ability to work independently and within a team

Enhanced DBS on the Update Service (or willingness to obtain one)
