Are you a passionate and experienced Social Worker with a background in hospital settings?
Wakefield Council is seeking a dedicated Senior Social Worker to join its Hospital Social Work Team, supporting patients and their families through the hospital discharge process and into appropriate post-discharge care.
About the Role:
As a Senior Social Worker, you will play a critical role in delivering high-quality support and advocacy for individuals within a fast-paced hospital environment. You will work closely with multidisciplinary teams to ensure safe and timely discharges, while promoting the rights, dignity, and independence of each individual.
This role also includes mentoring junior staff, managing complex cases, and contributing to the development of best practices across the team.
Key Responsibilities:
Discharge Planning & Assessment: Manage a caseload of patients requiring social work input to facilitate safe and appropriate discharges
Multi-agency Collaboration: Work in partnership with health professionals (e.g., doctors, nurses, OTs) to coordinate care and support
Team Leadership: Provide supervision and mentoring to less experienced colleagues support a culture of learning and development
Safeguarding & Advocacy: Ensure the patient's voice is central in care planning uphold safeguarding principles in every decision
Crisis Management & Complex Cases: Respond flexibly and effectively to urgent situations and high-risk cases
What We're Looking For:
Qualified Social Worker registered with Social Work England
Minimum 3 years' experience, preferably in a hospital or health-based setting
Proven ability to manage complex cases and discharge planning
Experience providing leadership, supervision, or mentoring to team members
Strong understanding of the Care Act, Mental Capacity Act, and safeguarding frameworks
Excellent interpersonal and communication skills with a person-centred and strengths-based approach

The team undertakes statutory safeguarding work, including child protection, child in need, and care proceedings. The successful candidate will play a vital role in delivering high-quality, child-focused social work practice in line with legislative and policy frameworks.
Key ResponsibilitiesManage a caseload involving children subject to CIN and CP plans
Undertake Section 47 enquiries, assessments, and direct work with families
Initiate and progress care proceedings where necessary
Build strong, trust-based relationships with children and their families
Contribute to core group meetings, child protection conferences, and reviews
Work collaboratively with health, education, police, and other partners

RequirementsSocial Work qualification (BA/MA Social Work, DipSW or equivalent)
Registration with Social Work England
Minimum of 2 years' post-qualification experience in statutory safeguarding work
Experience of working within busy frontline safeguarding teams
Strong communication, risk assessment, and analytical skills
Enhanced DBS registered to the Update Service (or willingness to obtain)
Ability to work under pressure and meet statutory timescales
Driving licence and access to a car for visits is desirable
