This is a fantastic opportunity for an experienced adult social worker who is passionate about supporting adults with learning disabilities to live as independently and safely as possible in their communities.

Carry out Care Act assessments, reviews, and support planning.
Undertake Mental Capacity Assessments and best interest decisions.
Work collaboratively with health professionals, families, and providers.
Safeguard adults at risk in line with statutory duties.

Qualified Social Worker - Adult Neurodiversity Team

We are currently looking to recruit 2 experienced Qualified Social Workers to join Worcestershires Adult Neurodiversity Team - a developing and innovative team committed to supporting adults with neurodivergent needs.
About the Team:
The Adult Neurodiversity Team is a newly established service made up of Social Workers, Social Care Workers, and a Housing Officer. The team is supported by an Advanced Social Work Practitioner and a Team Manager.
The service supports adults with autism, ADHD, learning disabilities, and other associated conditions. The team works under a strengths-based model, focusing on creating personalised care plans that promote independence, choice, and overall well-being.
Key Responsibilities:
Undertaking Care Act assessments and reviews
Completing Mental Capacity Assessments
Leading and contributing to safeguarding enquiries
Developing outcome-focused support plans
Working closely with individuals, families, and other professionals
Requirements:
Registered with Social Work England
Proven experience working with neurodivergent adults and their families
Strong understanding of autism, ADHD, and learning disabilities
Experience with capacity assessments, safeguarding, and person-centred planning
Ability to work independently and as part of a multi-disciplinary team

Team: Young Adults Team - All Age Disability Service
About the Role:
Worcestershire County Council is seeking a Qualified Social Worker to join the All Age Disability Service, specifically supporting young adults aged 16-25 through the transition from Children's to Adult Services. The successful candidate will bring specialist skills across both Adult and Children's Services, focusing on preparation for adulthood across education, employment, independent living, health, and community inclusion.
Key Requirements:
Must have experience in transitions work
Confident in navigating both children's and adult legislation
Ability to work in a multi-agency environment, including health, SEND, and education
Proficiency with Liquidlogic systems (children and adult)
Full UK driving license and willingness to travel across Worcestershire (and occasional national placements)
