Herefordshire Adult Social Care is looking for an experienced CHC Senior Practitioner to undertake Joint Assessments / Reviews with our ICB to assess eligibility for CHC and Joint Funding. Must have a strong working knowledge of eligibility and process under the 2022 National Framework for NHS Continuing Healthcare and the Care Act 2014. Experience of undertaking CHC Checklists, Decision Support Tools and undertaking challenges and disputes regarding CHC Eligibility and Joint Funding with the ICB.
You will be familiar DST domains and experienced in obtaining evidence to support presenting argument based on Nature, Intensity, Complexity and Unpredictability of health need. You will also have excellent communication, presentation and negotiation skills.
Herefordshire Adult Social Care uses the Mosaic client database so experience with this system would be an advantage, but training will be provided as required. You will support the CHC Lead with the overall management of the CHC pathway from referral to outcome and monitoring the financial implications of those outcomes.
We are currently working a hybrid system of part office/part home-based working. There is a requirement to be able to work face to face and be based in the office in Hereford for at least 2 days each week. You will be given autonomy in the booking and confirming of visits for the days in office and then writing-up on the work from home days.

The Childrens Safeguarding and Support teams are based in the centre of Hereford with good transport links to get around the City as needed. The staff are very welcoming and are always keen to offer help and guidance if needed to anyone new to the team.
We work alongside families in improving outcomes for their children, we have a restorative approach to social work and this is also evident in the team culture and senior management style as we are always available for discussions on complex matters which we work together to ensure the right outcome is achieved for our families.
The Social Workers work closely with the Council's Legal Team to complete Court applications this can be a learning experience for Social Workers but plenty of support is available to help when needed, we have court progression officers who support the teams in ensuring we meet court timescales and identify any training that may be required to ensure that our staff feel equipped & supported.
Essential Requirements:
* Social Work Qualification - Degree or equivalent
* At least 3 years' experience as a social worker within children's services
* Social Work England Registration
* Driving license and access to own vehicle

You will be fully qualified (DipSW, CQSW, CSS or equivalent), with a minimum of 3 years' experience in statutory social care. With honed skills in assessment, report writing, and managing a complex caseload, you will be dedicated to improving the lives of children and families within Herefordshire.
45p per mile Business Mileage - £150 per week accommodation expense

About the Role:
Herefordshire Council's Adults & Communities Care Act Assessment Team is seeking an experienced Locum Senior Practitioner Social Worker. You'll be supporting individuals recently discharged from hospital through Discharge to Assess, providing short-term assessment and support planning for those requiring ongoing services.
Key Responsibilities:
Lead assessments and support planning in line with the Care Act
Supervise, guide and support staff daily
Oversee safeguarding, case allocations, triaging, and assessments
Contribute to timely discharges and continuity of care
Manage a small caseload where necessary
What We're Looking For:
Qualified Social Worker registered with Social Work England
Strong experience in adult social care and hospital discharge pathways
Working knowledge of CHC, MCA (2005), mental health, LD, physical disabilities, and older adults
Proven ability to make sound decisions under pressure and manage risk effectively
