We are currently seeking an experienced and proactive Qualified Social Worker (QSW) to join the Hospital Discharge Team at Barnet General Hospital, supporting discharge planning for residents of the London Borough of Enfield (LBE).
Key Responsibilities:
Facilitate safe and timely discharges from hospital
Complete Care Act assessments and support planning
Undertake Mental Capacity Assessments and Best Interest decisions as required
Work collaboratively with health professionals, families, and external providers
Ensure appropriate support packages are in place for discharged individuals
Maintain accurate case notes using the council's systems
Requirements:
Qualified Social Worker with Social Work England registration
Minimum of 2 years' experience in Adult Social Care
Previous hospital discharge or similar experience preferred
Strong understanding of Care Act 2014, MCA, and NHS discharge processes
