We are seeking a dedicated and passionate Qualified Social Worker (QSW) to join our South Neighbourhood Team in Stockport. As a QSW, you will play a vital role in assessing needs, providing support, and ensuring the welfare of vulnerable populations.

Your key responsibilities will include:

Conducting comprehensive assessments of children, families, and adults to identify their needs and strengths. Developing and implementing tailored care plans in collaboration with service users and other professionals. Maintaining accurate and up-to-date case records in accordance with statutory requirements and best practice guidelines. Providing support and guidance to families, ensuring they have access to appropriate resources and services. Working collaboratively with multi-agency teams to promote the welfare and safety of vulnerable individuals.

To be considered for this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in a social work setting, preferably within a neighbourhood team. Strong assessment, analytical, and communication skills. The ability to work effectively under pressure and manage a varied caseload.

Key Responsibilities:
Oversee and supervise a team of social workers and unqualified staff within a busy hospital discharge service

Ensure effective and person-centred hospital discharges across all discharge pathways

Work across Stepping Hill Hospital, Regent House, and other bed-based sites

Lead and attend safeguarding enquiries, Court of Protection cases, MDT meetings, and other professional forums

Collaborate effectively with key partners including ICB, health services, and third-sector organisations

Ensure staff compliance with key legislative frameworks including the Care Act, MCA, BIA, and Safeguarding procedures

Requirements:
