Key Responsibilities:
Oversee and supervise a team of social workers and unqualified staff within a busy hospital discharge service
Ensure effective and person-centred hospital discharges across all discharge pathways
Work across Stepping Hill Hospital, Regent House, and other bed-based sites
Lead and attend safeguarding enquiries, Court of Protection cases, MDT meetings, and other professional forums
Collaborate effectively with key partners including ICB, health services, and third-sector organisations
Ensure staff compliance with key legislative frameworks including the Care Act, MCA, BIA, and Safeguarding procedures
Requirements:
Qualified Social Worker with Social Work England registration
Proven experience in supervision, team management, and complex discharge planning
Strong working knowledge of relevant legislation and social care policy
Ability to work under pressure in a fast-paced, multidisciplinary setting
Must be reliable, flexible, and resilient
Must hold a valid UK driving licence
