We are seeking a dedicated and compassionate Stoke - Social Worker to join our team in the Mental Health Acute Hospital discharge department. This role is pivotal in ensuring that patients receive the support they need during their transition from hospital to home. As a qualified social worker, you will work collaboratively with healthcare professionals, patients, and their families to facilitate smooth discharges and promote positive mental health outcomes.

Key responsibilities include:

Conducting comprehensive assessments of patients’ social and emotional needs. Developing and implementing individualized discharge plans in collaboration with multidisciplinary teams. Providing emotional support and counseling to patients and their families throughout the discharge process. Connecting patients with community resources and support services post-discharge. Monitoring and evaluating the effectiveness of discharge plans and making necessary adjustments.

Essential qualifications and skills for this role include:

A recognized degree in Social Work and a valid Social Work registration. Proven experience in mental health settings, particularly in acute hospital discharge. Strong communication and interpersonal skills, with the ability to build rapport with diverse populations. Excellent organizational skills and the ability to manage multiple cases effectively. Knowledge of mental health legislation and discharge planning processes.
