The role is part of the Unplanned Care services, supporting rapid response and early intervention therapy to prevent unnecessary hospital admissions and enable safe discharge into the community. The successful candidate will work across Crawley and Horsham, covering a wide area from Pulborough to Horley, providing timely and effective physiotherapy assessments and interventions in a community setting. Key Responsibilities: Undertake physiotherapy assessments in patients' homes or community settings. Deliver high-quality clinical interventions that support urgent response and reablement goals. Work closely within a multidisciplinary team to facilitate safe discharge and prevent avoidable hospital admissions. Provide ongoing monitoring, evaluation, and adjustment of treatment plans. Maintain up-to-date and accurate clinical documentation.
