We are seeking a dedicated and experienced Qualified Social Worker to join our Adult's Assessment Team in Lancashire. This role is pivotal in supporting adults in need, ensuring their voices are heard and their rights are protected. As part of a dynamic and collaborative team, you will conduct comprehensive assessments, develop care plans, and provide ongoing support to individuals and their families.
Key responsibilities include:
Conducting thorough assessments of adults’ needs, risks, and strengths. Developing and reviewing care plans in collaboration with service users and their families. Working closely with multidisciplinary teams, including health professionals and community services. Maintaining accurate and up-to-date case records in accordance with local policies and procedures. Providing guidance and support to less experienced team members.To be successful in this role, candidates must possess:
A relevant social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Experience in adult social work, particularly in assessments and care planning. Strong communication and interpersonal skills, with a compassionate approach to service delivery.
