Are you a passionate and dedicated social worker looking to make a meaningful impact in the community? We are seeking a Qualified Social Worker to join our Adult's Social Work Community Team in Barnet. In this role, you will work collaboratively with individuals and families to promote their well-being and empower them to lead fulfilling lives. Your expertise will be crucial in assessing needs, developing care plans, and providing ongoing support to vulnerable adults in our community.
Key responsibilities include:
Conducting comprehensive assessments of service users’ needs and risks. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing advice, support, and guidance to individuals and their families regarding available services. Maintaining accurate and up-to-date case records and documentation. Collaborating with other professionals and agencies to ensure a holistic approach to care. Participating in team meetings and contributing to service development initiatives.To be considered for this role, you must hold a relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent) and be registered with Social Work England. Additionally, candidates should possess:
Strong knowledge of adult safeguarding procedures and legislation. Excellent communication and interpersonal skills. Ability to work effectively under pressure and manage a diverse caseload.
