The London Borough of Merton is looking for a qualified Social Worker to join our Community/LD Team. In this role, you will play a crucial part in supporting individuals and their families, ensuring they receive the care and resources they need to thrive in the community.
As a Social Worker in our Community/LD Team, your key responsibilities will include:
Conducting comprehensive assessments of adults with learning disabilities to identify their needs and support requirements. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing ongoing support, guidance, and advocacy to individuals, ensuring their voices are heard and respected. Working closely with multidisciplinary teams, including healthcare professionals, to coordinate services and interventions. Maintaining accurate and up-to-date case records in compliance with legislation and local policies.To be considered for this role, you must possess the following essential qualifications and skills:
A relevant social work qualification (e.g., Degree in Social Work or equivalent). Current registration with Social Work England. Proven experience working with adults with learning disabilities or within a community setting. Strong assessment and analytical skills, with the ability to develop effective care plans.
