We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Barnet. This position offers an exciting opportunity to make a real difference in the lives of adults who require support during their hospital stay and transition back into the community. You will be an integral part of a multidisciplinary team, working collaboratively to assess, plan, and implement care for individuals with complex needs.
As a Qualified Social Worker, your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and risks. Developing and implementing care plans in collaboration with service users and their families. Providing emotional support and guidance to service users and their families throughout the hospital stay. Working closely with healthcare professionals to ensure a holistic approach to care. Facilitating safe discharge planning and connecting individuals to appropriate community resources. Maintaining accurate and up-to-date records in compliance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, CSS, or equivalent). Registration with Social Work England. Demonstrable experience in adult social work, particularly in a hospital setting. Strong communication and interpersonal skills, with the ability to work effectively in a team.
