We are seeking a dedicated and experienced Adult's Social Worker to join our Hospital Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults in hospital settings, ensuring their well-being and facilitating safe and timely discharges.

Key responsibilities and duties include:

Assessing the needs of adult patients in hospital and developing care plans in collaboration with multidisciplinary teams. Conducting safeguarding investigations and interventions to protect vulnerable adults. Supporting individuals in accessing appropriate community resources and services to promote independence and recovery. Advocating for patients' rights and best interests while adhering to legal and ethical standards.

The ideal candidate will possess:

A recognized qualification in Social Work and registration with Social Work England. Demonstrated experience in adult social work, particularly in hospital or healthcare settings. Strong assessment and communication skills, with the ability to engage effectively with diverse individuals and families. Knowledge of relevant legislation, policies, and procedures related to adult social care.

Additional requirements:

Valid driver's license and access to a vehicle for travel between hospital and community locations. Flexibility to work occasional evenings or weekends to meet the needs of patients and service demands.
