Key responsibilities and duties:

Conducting assessments to determine the needs and risks of adult patients Developing and implementing care plans tailored to individual needs Providing advocacy and support to patients and their families Collaborating with healthcare professionals to ensure coordinated care Participating in multidisciplinary meetings and case conferences

Essential qualifications and skills:

Qualified Social Worker with relevant experience in adult social care Registration with Social Work England Strong assessment and communication skills Ability to work effectively in a fast-paced and challenging environment Commitment to promoting the rights and dignity of vulnerable adults
