We are currently seeking a dedicated and experienced Adult's Social Worker to join our Long Term Planning Team in Luton. As a key member of our team, you will play a crucial role in supporting and safeguarding vulnerable adults in the community.

Key responsibilities and duties:

Conduct comprehensive assessments of adults in need of support and develop care plans accordingly Work collaboratively with other professionals and agencies to provide holistic care and support to service users Monitor and review care plans regularly to ensure they meet the changing needs of the individuals Participate in multi-disciplinary meetings and contribute to case conferences as required Provide advocacy and support to service users in navigating the social care system

Essential qualifications and skills:

Qualified Social Worker with a valid HCPC registration Demonstrated experience in working with vulnerable adults in a social care setting Strong assessment and analytical skills, with the ability to make informed decisions Excellent communication and interpersonal skills to engage effectively with service users and stakeholders Ability to work autonomously and as part of a team in a fast-paced environment
