We are seeking a dedicated and compassionate Qualified Social Worker to join our Older Peoples Team in Wokingham. This role is vital in supporting adults aged 65 and over, ensuring they receive the highest standard of care and support. You will be responsible for conducting assessments, developing care plans, and collaborating with various stakeholders to promote the well-being and independence of older adults in our community.

Your key responsibilities will include:

Conducting comprehensive assessments of the needs of older adults and their families. Developing and implementing tailored care plans in collaboration with service users and their support networks. Providing ongoing support and monitoring of care plans to ensure effective service delivery. Working closely with health professionals, community organizations, and other social services to coordinate care. Advocating for the rights and needs of older adults, ensuring their voices are heard in decision-making processes.

To be considered for this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Demonstrable experience in adult social work, particularly with older populations. Strong assessment and care planning skills, with an understanding of safeguarding procedures. Excellent communication and interpersonal skills, with the ability to build rapport with service users and their families.
