Social Workers required as an Advanced Practitioner for work within the Adult Acute Hospital Team in the Yeovil area of Somerset. Excellent rates of pay up to £44ph. 3 month minimum contract but possibility to extend.37 hours per week. Flexible hybrid working arrangements.

This is an excellent opportunity to use your expertise in a fast-paced hospital setting, supporting safe and timely discharges while providing leadership and guidance to colleagues.

The Role

As an Advanced Practitioner, you will:

Manage a complex caseload, undertaking high-quality assessments and interventions.
Support adults and their families to achieve positive outcomes through person-centred, strengths-based practice.
Work collaboratively with NHS colleagues and multi-agency partners to facilitate safe hospital discharges.
Provide practice leadership, mentoring, and support to Social Workers within the team.
Promote high standards of safeguarding, assessment, and care planning.
Contribute to service development and help drive continuous improvement.

You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
