Eden Brown Synergy is currently recruiting for an experienced Occupational Therapist to join the Reablement Team at Brent Council.

This is an excellent opportunity to work within a supportive multidisciplinary team, helping residents maximise their independence following hospital discharge or referral through community pathways.

About the Role

You will work collaboratively with Social Workers, Care Assessors, Enablers, health professionals, hospital teams, and partner agencies to deliver person-centred, strengths-based interventions that support individuals to regain and maintain their independence.

Key responsibilities include:

Undertaking strengths-based Occupational Therapy assessments.Supporting residents following hospital discharge and through community referrals.Recommending and arranging appropriate equipment and adaptations.Completing home visits and functional assessments across the borough.Working closely with multidisciplinary professionals to deliver coordinated care.Promoting independence and preventing the need for ongoing care and support.Maintaining accurate case records and documentation in line with professional standards.Essential RequirementsHCPC Registered Occupational Therapist.Previous experience within Adult Social Care and/or Reablement services.Strong knowledge of strengths-based practice and person-centred assessment.Experience completing community-based assessments, home visits, and equipment recommendations.Excellent communication and multidisciplinary working skills.Enhanced DBS (or willingness to obtain one).Working Arrangements

The team is based at Brent Civic Centre. Practitioners are required to work within the borough for a minimum of three days per week to support effective partnership working and service delivery. This may include working from customers' homes, community settings, partner locations, or the Civic Centre.
