Job Opportunity: RNLD/RMN - 3-Month Contract

Pay Rate: £320pd

We are supporting a client in the London area with a S117 Joint Funding project that requires additional resource. This is a hybrid role although you will need to be available for face to face visits.

Day to Day Duties:

Deliver care plans and interventions.Collaborate with multi-disciplinary teams, including social workers and healthcare professionals.Monitor and assess client progress, ensuring their health and well-being.Provide support with activities of daily living, promoting independence and choice.Conduct risk assessments and manage care-related documentation.
