Join our dedicated team as a Qualified Social Worker (QSW) in Older Adults Mental Health in Coventry, where you will play a crucial role in enhancing the lives of older adults experiencing mental health challenges. We are seeking an enthusiastic and compassionate professional who is committed to providing high-quality support and intervention to individuals and their families. In this role, you will work collaboratively with multidisciplinary teams to assess needs, develop care plans, and facilitate access to necessary services.
Your key responsibilities will include:
Conducting comprehensive assessments of mental health needs for older adults. Developing and implementing person-centered care plans in collaboration with clients and their families.
