We are seeking a skilled Adult Social Worker to join our team, providing high-quality support to older adults with complex needs. You will work closely with individuals, families, and multidisciplinary teams to assess needs, develop care plans, and promote independence.

Key Responsibilities:

Conduct Care Act assessments and reviews for older people

Carry out Mental Capacity and Best Interest assessments

Manage safeguarding enquiries and develop safeguarding plans

Collaborate with health, social care, and community partners

Prepare reports and contribute to court proceedings when required

Requirements:

Qualified Social Worker (HCPC registered)

Experience working with older adults in social care

Strong knowledge of relevant legislation and safeguarding policies

Excellent communication and case management skills

Ability to work independently and as part of a multidisciplinary team
