We are currently seeking a dedicated and compassionate Qualified Social Worker to join our Reablement Service team in Islington. As a key member of the team, you will be responsible for providing support and assistance to individuals in the community who are working towards regaining their independence and improving their quality of life.

Key responsibilities and duties:

Conduct assessments to identify individual needs and develop personalized care plans Provide practical and emotional support to service users to promote independence Work collaboratively with other healthcare professionals, families, and carers to ensure holistic care Monitor progress and adjust care plans as needed to achieve reablement goals Maintain accurate and up-to-date records of all interactions and interventions

Essential qualifications and skills:

Qualified Social Worker with a valid registration with the Health and Care Professions Council (HCPC) Experience working in a similar role, preferably within a reablement or community setting Excellent communication and interpersonal skills Ability to work effectively in a fast-paced and challenging environment Strong assessment and care planning skills
