An Exciting Opportunity in Adult Social Services - Join the Hospital Discharge Team!

We are currently recruiting an experienced and motivated Social Worker to join the Adult Social Services Team based at the Norfolk and Norwich University Hospital. This role is perfect for those with a passion for complex casework and experience working as part of a multi-disciplinary team.

Role Overview:
You will be supporting inpatients and facilitating safe, timely discharges from hospital. The role is varied, fast-paced, and involves working in close partnership with a wide range of health professionals and providers. This is an excellent opportunity to develop your skills in a supportive, collaborative environment.

Key Responsibilities:

Carry out Care Act assessments and Discharge to Assess planning

Conduct Mental Capacity Assessments

Lead on Safeguarding enquiries

Maintain a caseload supporting safe discharge from hospital

Work effectively within a multi-disciplinary team (MDT)

Ensure person-centred and strengths-based approaches to care planning

Essential Requirements:

Social Work qualification (MA, BA Social Work, DipSW or CQSW)

Registration with Social Work England

Recent experience in hospital discharge or similar adult social care settings

Strong working knowledge of the Care Act and associated legislation

Ability to work on-site at Norfolk and Norwich University Hospital

Working Arrangements:

Office-based role - applicants must be able to attend the hospital in person

Interviews will be conducted via video technology
