* Work within the Transition Team, which sits alongside the Learning Disabilities Team, ensuring coordinated and effective support for young adults.
* Complete Care Act transition assessments, support planning, reviews, and risk assessments to promote positive outcomes.
* Develop person-centred transition plans in partnership with young people, families, schools, colleges, health professionals, and partner agencies.
* Promote independence and maximise opportunities for young adults through strengths-based practice and effective multi-agency working.
* Undertake safeguarding enquiries, Mental Capacity Act assessments, and Best Interest decisions where required.
* Maintain accurate case records and ensure all work is completed in accordance with relevant legislation, policies, and best practice guidance.

Essential Requirements:
* Social Work Qualification - Degree or equivalent.

* Significant post-qualified experience within Adult Social Care, Transition Services, or Learning Disabilities.
* Previous experience supporting young people aged 18-25 through transition into Adult Services.
* Experience working with individuals with learning disabilities and/or other disabilities.
* Good working knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and transition planning.
* Strong communication skills and the ability to work effectively within a multi-agency environment.
