An excellent opportunity has arisen for an experienced Qualified Social Worker to join the Integrated Discharge Team (Contact Cares) based at Whiston Hospital, working to support timely and safe hospital discharges.
You will play a vital role in assessing patients and coordinating discharge plans that enable individuals to return home or transition to appropriate care settings. This is a fast-paced and high-pressure environment, and previous experience in hospital discharge or similar settings is highly desirable.
Key Responsibilities:
Undertake timely assessments of patients to facilitate safe, effective discharge planning.
Collaborate with NHS professionals, therapy teams, and community services to ensure a multi-agency approach.
Arrange appropriate support packages to meet patient needs post-discharge.
Support discharges to home, reablement services, or long-term care placements, as appropriate.
Complete relevant documentation and adhere to statutory responsibilities and best practice.
Attend ward rounds and liaise with hospital teams as part of daily duties.
Operate on a hybrid working pattern, including office-based, ward-based, and home-based work on a rota.
Requirements:
Social Work England registration (essential)
Minimum of 2 years' post-qualifying experience, ideally in hospital discharge or adult social care
Sound knowledge of the Care Act, Mental Capacity Act, and discharge planning protocols
Ability to work in a fast-paced, multi-disciplinary environment
Strong organisational and communication skills
Must be able to commit to a full-time 37-hour week

We are seeking a qualified and experienced Approved Mental Health Professional (AMHP) to join the St. Helens team on a temporary basis. You'll be working as part of the AMHP rota to carry out Mental Health Act Assessments across various settings, including community and hospital environments.
Due to ongoing staff shortages from sickness, annual leave, and vacant positions, your support is urgently needed-especially over the Christmas and New Year period.
Key Responsibilities:
Undertake Mental Health Act Assessments in line with the Mental Health Act 1983
Work flexibly across all settings as needed
Provide statutory input as required under AMHP duties
Be available to cover shifts during the festive period
Work outside of normal office hours if necessary, depending on assessment demands
Requirements:
Qualified AMHP (Approved Mental Health Professional)
Extensive experience in Mental Health Act Assessments
Flexibility to work during the Christmas/New Year period
Willingness to work outside normal hours if required
