We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) for a Hospital Discharge position in Wiltshire. In this role, you will play a crucial part in facilitating safe and timely discharges for patients, ensuring they receive appropriate support and services to transition smoothly back into the community.
Key responsibilities and duties:
Conducting assessments to determine the needs of patients being discharged from the hospital. Developing and implementing care plans in collaboration with the multidisciplinary team to support patients post-discharge. Liaising with external agencies, such as community services and housing providers, to arrange ongoing support for patients. Advocating for patients to access necessary resources and services to promote their well-being. Providing emotional support and guidance to patients and their families during the discharge process.Essential qualifications and skills:
Qualified Social Worker with a valid registration with Social Work England. Previous experience working in a hospital or healthcare setting, preferably in a discharge planning role. Strong assessment and care planning skills with the ability to work collaboratively with other professionals.
