Darlington Borough Council is seeking an experienced and motivated Adults Social Worker to join our team, supporting timely and person-centred hospital discharges. This is a key role that contributes to safe transitions from hospital to community settings, working closely with health and care partners.
Pay: £26.61 per hour / £196.95 per day
Role Overview:
The role focuses specifically on:
Managing and facilitating hospital discharges
Reviewing individuals in temporary residential placements following discharge
Working within a Home First model
Applying a strengths-based approach to assessment and care planning
Ensuring safe, effective, and timely transitions aligned with Care Act duties
Requirements:
Qualified Social Worker with SWE registration
Proven experience in adult social care, ideally within hospital or discharge settings
Strong understanding of the Care Act, Home First principles, and strengths-based practice
Ability to work efficiently in a fast-paced, multidisciplinary environment
Excellent communication and assessment skills
Contract Details:
Full-time hours (37 hours/week)

Priory Hospital Middleton St George is seeking a highly motivated and experienced Lead Occupational Therapist to lead our Occupational Therapy service. This is an exciting opportunity to play a central role in shaping and delivering person-centred, recovery-focused care across a range of specialist mental health services, including acute, PICU, and rehabilitation settings.
Key Responsibilities:
Provide professional leadership to the OT team and wider therapy services
Oversee the development and delivery of evidence-based OT interventions
Ensure high-quality assessments and personalised treatment planning
Supervise and support the development of junior OTs and OT assistants
Collaborate closely with the wider MDT to promote holistic recovery outcomes
Lead on service audits, outcome measures, and continuous service improvement initiatives

Requirements:
HCPC-registered Occupational Therapist

We are seeking a dedicated and skilled Qualified Social Worker (QSW) to join our dynamic Adult Social Care Team in Darlington.

Role Overview:
As part of the Adult Social Care Team, you will:
Undertake statutory assessments including Care and Support Needs Assessments, Mental Capacity Assessments, Safeguarding Enquiries, and Support Planning
Promote independence and well-being through a strengths-based approach
Work collaboratively with service users, families, carers, and partner professionals
Ensure compliance with the Care Act 2014 and other relevant legislation
Requirements:
