Job Title: Band 6 Specialist Physiotherapist
Location: Cross Site - Community Therapy Team West / Community Therapy Team WS - North
Base: Rose Wing, Horsham Hospital
Hours: 4 days per week (flexible on which 4 days)
Contract: Locum / Temporary
Overview:
We are currently seeking a dedicated and experienced Band 6 Specialist Physiotherapist to join the Community Therapy Team West. This is a fantastic opportunity to deliver high-quality physiotherapy interventions within a supportive multidisciplinary environment, working across the community setting in West Sussex.
Key Responsibilities:
* Provide specialist physiotherapy assessment, treatment, and management for a wide range of patients in the community.
* Work as part of a multi-disciplinary team to develop personalised care plans and rehabilitation programmes.
* Support hospital discharge and prevent hospital admission through timely, goal-oriented physiotherapy interventions.
* Liaise effectively with GPs, community nursing teams, and social care colleagues to ensure seamless patient care.
* Maintain accurate clinical records and contribute to service quality improvement activities.
* Participate in the supervision and support of junior staff and students.
Requirements:
* HCPC registered Physiotherapist.
* Previous experience working at Band 6 level in a community physiotherapy setting.
* Excellent assessment and clinical reasoning skills.
* Full UK Driving Licence and access to a vehicle (essential).
* Smart Card (essential).
* SystmOne experience (desirable).
Additional Information:
* This is a cross-site community role with a base at Rose Wing, Horsham Hospital.
* The role is offered on a 4-day per week basis flexible days to suit candidate availability.
