We are currently seeking a passionate and dedicated Adult's Social Worker to join our Multi-Disciplinary Team in Cornwall. As a key member of the team, you will be responsible for providing high-quality social work services to adults in need, working collaboratively with other professionals to ensure the best outcomes for vulnerable individuals.

Key responsibilities and duties:

Conducting assessments and developing care plans for adults with complex needs Providing support and guidance to service users and their families Working closely with healthcare professionals, mental health teams, and other agencies to coordinate care and support Participating in case conferences and reviews to evaluate progress and make necessary adjustments to care plans Advocating for the rights and best interests of service users, ensuring their voices are heard and respected

Essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Work England Demonstrated experience in working with adults in a social care setting Excellent communication and interpersonal skills Strong assessment and decision-making abilities Ability to work effectively within a multi-disciplinary team
