Key responsibilities and duties include:

Conducting assessments to determine the needs of children with disabilities and developing care plans accordingly Working collaboratively with multi-disciplinary teams to ensure the best outcomes for children and families Providing advocacy and support to empower children and families to access appropriate services and resources Participating in case conferences, reviews, and court proceedings as required Ensuring all work is in line with relevant legislation, policies, and procedures

The ideal candidate for this position will possess:

A degree in Social Work and registration with Social Work England Proven experience working with children with disabilities and their families Excellent communication and interpersonal skills Strong assessment and analytical abilities Ability to work effectively under pressure and manage a complex caseload
