We are currently recruiting for two experienced Adult Social Workers to join Norfolk County Council hospital discharge services, based at:
Norfolk and Norwich University Hospital (NNUH), NR4 7UY - Acute Hospital Discharge TeamWillow Therapy Unit, Norwich Community Hospital, NR2 3TU - Community Hospital / Rehab Discharge TeamBoth roles are available for an immediate start on a 6-month contract.
These are exciting opportunities to work within fast-paced multidisciplinary hospital environments, supporting safe and timely discharge through a Home First approach, promoting independence, recovery, and person-centred care.
