We are currently seeking a dedicated and experienced Warrington - Adults Advanced Practitioner to join our Community Mental Health Team (CMHT) on a contract basis, offering a competitive rate of £32 per hour. As a key member of the team, you will play a crucial role in providing high-quality social work support to adults with complex mental health needs.
Key responsibilities and duties:
Undertake comprehensive assessments of service users' needs and develop individual care plans. Provide specialist interventions and support to promote recovery and independence. Work collaboratively with multi-disciplinary teams and external agencies to ensure holistic care delivery. Conduct safeguarding investigations and take appropriate actions to protect vulnerable adults. Participate in case conferences, reviews, and team meetings to contribute to service development and improvement.Essential qualifications and skills:
Qualified Social Worker with current registration with Social Work England. Demonstrable experience working within a community mental health setting with adults. Strong assessment, risk management, and care planning skills. Excellent communication and interpersonal abilities to engage effectively with service users, families, and stakeholders. We are looking for an experienced Adults Advanced Practitioner to join the Working Age Community Mental Health Team (CMHT) in Warrington. This role is ideal for a skilled and motivated social worker who thrives in a collaborative, multidisciplinary setting and is committed to delivering high-quality care and support.
Key Responsibilities:
Complete Care Act assessments
Arrange and monitor care packages to ensure their ongoing effectiveness
Monitor mental health and support individuals in maintaining their well-being
Work as part of a multi-disciplinary team (MDT) alongside health professionals to manage risk
Record accurate and up-to-date case notes on the Paris system
Manage safeguarding responsibilities and purchase care packages using Mosaic
Requirements:
Social Work qualification (CQSW, DipSW, CSS, or equivalent)
Registered with Social Work England
Substantial post-qualifying experience in adult mental health
Familiarity with Paris and Mosaic recording systems
Strong understanding of Care Act legislation, safeguarding, and risk management
Ability to work effectively both independently and within a team
