Social Worker - Sensory, Physical & Neurological Team
Warrington Borough Council
£30 per hour
We are recruiting for an experienced Adult Social Worker to join the Sensory, Physical & Neurological Team at Warrington Borough Council.
This role involves supporting adults with sensory impairments, physical disabilities, neurological conditions, autism, and complex needs to achieve positive outcomes and promote independence.
Requirements:
Qualified Social Worker registered with Social Work England
Experience working within Adult Social Care
Experience supporting adults with autism and complex needs
Strong assessment and care planning skills
Excellent communication and partnership working skills
Location: 1 Time Square, Warrington, WA1 2NT
Pay Rate: £30 per hour

Adults Social Worker - Hospital Team (Transfer of Care Hub)
Warrington Hospital
£30 per hour
Full-time (37 hours per week)
Warrington Borough Council is seeking 2 experienced Adults Social Workers to join the Transfer of Care Hub at Warrington Hospital.
This is a fast-paced hospital discharge role supporting safe and timely discharges, working closely with health and multidisciplinary teams to reduce delayed transfers of care.
Key responsibilities:
Support hospital discharge planning and coordination
Undertake Care Act assessments and reviews
Work closely with health and MDT partners
Reduce delayed transfers of care through timely interventions
Ensure safe and appropriate discharge pathways
Requirements:
Recent experience in hospital or Transfer of Care settings
Strong assessment, communication, and decision-making skills
Knowledge of Mosaic system desirable
Qualified Social Worker with SWE registration
Working pattern:
Full-time, 37 hours per week
Hybrid role with approximately 3 days per week office-based at hospital

Key responsibilities include:
Conducting thorough assessments of the social, emotional, and practical needs of adults admitted to the hospital. Developing and implementing individualized care plans in collaboration with clients, families, and other professionals. Providing information and support to clients and their families regarding available services and resources. Facilitating discharge planning and ensuring smooth transitions to community services or residential placements. Maintaining accurate and up-to-date records in compliance with regulatory standards and organizational policies.The ideal candidate will possess the following qualifications and skills:
A relevant social work qualification (Degree in Social Work or equivalent). Registration with Social Work England. Experience in adult social work, preferably within a hospital or healthcare setting. Strong assessment and analytical skills with the ability to make sound decisions under pressure.
